Experienced Nurse Educators will conduct workshops on:
i. Living Well With Diabetes: Taking Care of Yourself
ii. Managing Your High Blood Pressure and Blood Cholesterol
iii. Weight Management and Reducing Your Risk of Diabetes

The workshops provide individuals with chronic disease (high blood pressure, diabetes, high blood cholesterol) and individuals at risk (BMI = 27.5kg/ m2 or have Impaired Glucose Tolerance (IGT)) with the necessary knowledge and skills to better manage their chronic conditions.

These will be conducted at 6 community centres/clubs and cost $10 for each workshop:

• Jalan Besar CC
• Kebun Bahru CC
• MacPherson CC
• Marine Parade CC
• Punggol Park CC
• Ulu Pandan CC

You can ask/ register for the workshops at
i. Chronic Disease Management Programme (CDMP) - registered GP clinics
ii. Wellness Coordinator of the Wellness Programme
iii. Grassroots leaders at the participating RCs/ CCs

All completed referral forms are to be faxed to the Health Promotion Board at 6538-8416 or sent via email to hpb_nurseeducator@hpb.gov.sg
