My Health Journal is an interactive tool that will help you to assess your risk for certain diseases and evaluate some of your lifestyle practices. Do take a few minutes to carry out the assessments. You will be given useful personalised advice to help you to focus on your prevention efforts.

My Health Journal provides 2 modes of access:

  • Via SingPass login - When you log in with your Singpass ID, you will be able to monitor the results of your assessments over time, i.e. you will be able to view your current as well as past results and monitor your risk over time.
  • Via Non-Login method - You will only be able to get the results of the current assessment. You will not be able to view past results and monitor your results over time.
