Health Promoting Health Services (HPHS) - Integrating Health Promotion & Preventive Health into Clinical Care

With Singapore’s rapidly ageing population and increasing chronic disease burden, health services are re-orienting themselves to meet not only the needs of the unhealthy, but also the at-risk population, to encourage our people to age well in place.

To better integrate and align national preventive health efforts, HPB initiated the concept of Health Promoting Health Services (HPHS), a coalition of health services sharing the vision of better health for all through primary prevention.  HPHS aims to improve the health of patients, staff and community by providing leadership and guiding principles to support health service providers to improve population health outcomes through coordinated evidence-informed strategies and programmes. The objective is to prevent or delay the on-set of diseases, to reduce the burden on the healthcare system.

HPHS Framework

The HPHS Framework emphasises the importance of partnerships, both within the HPHS Network and across the community and workplaces, to bring health promotion initiatives to Patients, Staff and the Community.
