What is HealthTrack SG about?

HealthTrack SG combines input from your wearable technology, clinical records and self-reported lifestyles to give you feedback on how you are progressing in terms of managing your chronic condition and overall wellbeing.

Improving lives and communities through digitally-powered care: A partnership between Health Promotion Board, Google, ConnectedLife and Fullerton Health, HealthTrack SG is a digital health programme that facilitates collaboration with your doctor and improves your health. As participants, you’re able to:

View your Wellness Score to gain insights into how your lifestyles affect your overall wellness, which areas you are doing well, and where you need to work on to improve your health.

Eligibility

  • Singapore Citizens or Permanent Residents aged 25-75

  • Have any of the following:

    1. Hypertension (High Blood Pressure) or Borderline High Blood Pressure

    2. Type 2 Diabetes Mellitus or Pre-Diabetes

    3. Lipid Disorders / High Cholesterol

    4. High Body Mass Index (BMI) ≥ 23 kg/m²

    5. High Waist Circumference

      1. Female ≥ 80 cm, Male ≥ 90 cm

  • Comfortable using a smartphone and wearing a health tracker

  • Comfortable with spoken and written English
