A. General information
1. What is HealthTrack SG?
HealthTrack SG is a year-long pilot programme conducted jointly by the Health Promotion Board, Google, ConnectedLife, and Fullerton Health. Through wearable device data and self-reported information, you can monitor your lifestyle habits (e.g. step count, physical activity, sleep) and health data (e.g. heart rate, blood pressure). The information is integrated in the HealthTrack SG app, and a Wellness Score is calculated to give an indication of your overall wellness and areas for improvement.
Your doctor can also view your Wellness Score and assess your lifestyle and clinical data to co-develop a personal Wellness Plan with specific lifestyle recommendations for you. By analysing your profile, the app will also provide tailored support through behavioural nudges and tips.
2. What is the difference between HealthTrack SG and Healthier SG?
Healthier SG is a national initiative by the Ministry of Health (MOH) to encourage Singaporeans to take proactive steps to manage their health, prevent the onset of chronic diseases, and have strong support to lead healthier lifestyles. Upon enrolling in Healthier SG with your preferred family clinic that will provide care for your health needs, your doctor will develop a personalised Healthier SG Health Plan to help improve your health.
HealthTrack SG complements Healthier SG by supporting you beyond clinic visits with behavioural nudges and tips. Through HealthTrack SG's app, you can track your daily lifestyle and health data, receive practical health tips (e.g. how to read food labels), and share insights with your doctor. This gives your doctor a more complete picture and helps in the personalisation of a Wellness Plan.
3. How long is the programme?
HealthTrack SG is a 1-year programme.
B. Enrolment & participation
4. Am I eligible to join HealthTrack SG?
You are eligible if you meet the following criteria:
Singapore Citizen or Permanent Resident between the age of 25 to 75 years old
Have been told by a healthcare provider to have any of the following conditions:
High Blood Pressure (Hypertension) or High-normal Blood Pressure
Type 2 Diabetes Mellitus or Pre-Diabetes
Lipid Disorders / High Cholesterol
High Body Mass Index (BMI) ≥ 23 kg/m²l
High Waist Circumference
Female ≥ 80 cm, Male ≥ 90 cm
Comfortable with both spoken and written English
Using a smart phone with iOS 15 or Android 10 and above, compatible with HPB Healthy 365, HealthTrack SG and the relevant wearable companion App (e.g. Fitbit, Apple Health, Samsung Health or Garmin Connect)
Comfortable to wear a wearable device during daily activities and sleep
Assessed by doctor to be suitable to join programme
5. Can I still participate in HealthTrack SG if I have already enrolled in another clinic for Healthier SG?
You may still participate in HealthTrack SG if you have enrolled in Healthier SG.
6. How will I benefit from participating in HealthTrack SG?
By participating in HealthTrack SG for the entire 1-year duration, you can receive up to $140 in rewards. Refer to Section K for more details on rewards and incentives.
As part of the programme, you would be required to use a wearable device to collect your health data. You will have access to individualised data and trends on your lifestyle habits (e.g. step count, physical activity, sleep) and health data (e.g. heart rate, blood pressure) through the HealthTrack SG app. You will also receive tailored support through in-app health messages and lifestyle recommendations from your healthcare provider based on your profile, lifestyle and preferences.
7. What should I do if I am interested to join HealthTrack SG?
Please complete the Eligibility Checker at https://www.healthtrack.gov.sg. If you are eligible, you will receive an eligibility email with a unique sign-up code to download the HealthTrack SG app and onboard the programme.
8. What do I need to onboard HealthTrack SG?
To onboard HealthTrack SG, you will need to:
Download HealthTrack SG app
Complete the Onboarding Survey on the HealthTrack SG app
Link your wearable device
Make a doctor's appointment* at a participating Fullerton Health clinic
Complete blood tests for glucose and cholesterol levels within 3 months of first doctor's consult

9. What is the commitment required of me if I am participating in HealthTrack SG?
The Programme requires active participation, which includes:
Wearing your wearable device during daily activities and sleep
Completing the Report Health Status every 2 weeks (Refer to Section I for more details on Report Health Status)
Interacting with the HealthTrack SG app regularly to monitor your Wellness Score
If there is no data from your wearable device or Report Health Status for more than 90 days, you will be automatically withdrawn from HealthTrack SG and your account will be terminated. Following programme withdrawal, future visits with your healthcare provider at Fullerton Health will not be part of the programme and you will no longer be eligible for rewards.
10. Can I choose to withdraw from HealthTrack SG at any time?
If you do not wish to continue with programme, you may withdraw by deleting your HealthTrack SG account.
On the HealthTrack SG app, tap the menu (≡) at the top left, then select Privacy & Data.
Click Delete Account to withdraw from the HealthTrack SG programme.
Please note that deleting the HealthTrack SG app will not lead to deletion of your account
Following programme withdrawal, future visits with your healthcare provider at Fullerton Health will not be part of the programme, and you will no longer be eligible for rewards.
11. Are there any costs involved in participating in HealthTrack SG?
Yes, participants are responsible for the costs of the consultation, blood tests, and your wearable device. Consultation and blood test fees follow Fullerton Health’s prevailing rates, while the cost of wearable device depends on the model selected. For model-specific discounts, please refer to Section D on Usage of Wearable Device (Health Tracker).
Prices may be revised without prior notice.
C. Use of HealthTrack SG app
12. How do I download the HealthTrack SG app?
You may download the HealthTrack SG app from the Google Playstore/Apple App Store. Upon completion of the eligibility checker located at https://www.healthtrack.gov.sg, eligible participants will receive a unique referral code to sign up for a HealthTrack SG account.
13. Which phone models are compatible with the HealthTrack SG app?
All phone models are compatible with the HealthTrack SG app except for the following:
Huawei P8 Lite
Huawei P9 Lite
Huawei P20 Lite
Xiaomi Mi 6
14. Do I need access to mobile data or internet connection during my participation in the programme?
Yes, access to the internet (e.g., Wi-Fi or mobile data) is required for the HealthTrack SG app to update your health data and access the app features. Internet access is not required when the app is running in the background.
15. Can I change my smartphone during my participation?
Yes. Remember to sync your wearable device with the HealthTrack SG mobile app before changing your smartphone to minimise the chances of data loss. You will be able to access your past data by downloading the HealthTrack SG app in your new smartphone and logging in with your credentials. You may refer to the eligibility email received prior to onboarding and follow the steps to setup your account on your new phone.
16. Who should I contact if I face any issues with the HealthTrack SG app?
If you are facing issues with the HealthTrack SG app, please refer to the in-app user guide for troubleshooting steps. On the HealthTrack SG app, tap on the customer support icon on the top right-hand corner, and select the User Guides. Refer to the relevant section for information.
If problems persist despite troubleshooting effort, please contact ConnectedLife's customer support at healthtracksg@support.connectedlife.io. Please provide detailed information about the issue, including any error messages encountered, to facilitate prompt resolution.
D. Usage of wearable device (Health tracker)
17. Which wearable health tracker are compatible with HealthTrack SG?
If you have an existing Fitbit, Apple, Samsung, Garmin or Google Pixel wearable device that is compatible with HealthTrack SG, you may use it to participate in HealthTrack SG.
For optimal programme experience, we recommend using a Fitbit Versa 3, Versa 4, Sense, or Sense 2.
If you do not currently own a Fitbit or a Google Pixel wearable device, you may purchase one at a discounted rate from the Google distributor website.

18. How do I sync my wearable device with the HealthTrack SG app?
First, ensure that your wearable device is connected to the respective health app (e.g. Fitbit device to be connected to Fitbit app).
After completion of the Onboarding Survey, you will be prompted to link your wearable device to the HealthTrack SG app. Click the Customer Support icon at the top right and go to User Guides. Refer to ‘Link and Delink Devices in the HealthTrack SG App’ .
19. Can I use the NSC tracker given by HPB?
The NSC tracker from HPB is not compatible with the HealthTrack SG app.
20. Who should I contact if I face any issues with my wearable device?

21. Can I change my wearable device during the programme?
Yes. Within the HealthTrack SG app, tap on 'My Devices' to delink your previous wearable device and link your new wearable device.
22. Can I use more than one wearable device for the same account?
No, you can only have one wearable device synced to your account at any one time.
E. Usage of Blood Pressure Monitor
23. Do I require a BP monitor?
Individuals with Hypertension (High BP) or high-normal BP require a BP monitor to ensure active participation in the programme. Regular BP measurements are required to help you monitor your health, receive important alerts, and allow your doctor to track your BP trends and work with you to create your Wellness Plan.
24. I already have a BP monitor. Do I need to purchase another one?
If you already have access to a BP monitor (e.g. at home or workplace), you may use your existing device to monitor your BP. Otherwise, you may choose to purchase either a Bluetooth-enabled BP monitor or a non-Bluetooth-enabled BP monitor.
25. How do I record my BP readings?
For Bluetooth-enabled Omron BP monitor
BP readings taken on a Bluetooth-enabled Omron BP monitor will be synced via Bluetooth to the Omron app. If the Omron app is linked to the HealthTrack SG app, these BP readings will also be automatically synced with the HealthTrack SG app for calculation of the Wellness Score.

For non-Bluetooth-enabled BP monitor
If you are using a non-Bluetooth-enabled BP monitor, please input your BP readings into the HealthTrack SG app to update your Wellness Score.
On the HealthTrack SG app, go to 'Workspace', navigate to 'Health Logs' and select 'Blood Pressure'. Click 'Add measurement' to fill in your Systolic BP (larger number), Diastolic BP (smaller number) and click 'Save'.

26. How frequently do I need to measure my BP?
Check your BP at least once a week if you’ve high or borderline high BP. If you have just started medications or are adjusting your treatment, more frequent monitoring might be needed. Seek advice from your doctor on the best BP monitoring schedule for you. Remember to log your BP readings in the HealthTrack SG app whenever you measure your BP.
F. Onboarding Survey
27. What is the Onboarding Survey?
You will need to complete an Onboarding Survey on the HealthTrack SG app as part of the onboarding process. The Onboarding Survey collects information on your demographics, medical condition, clinical data, lifestyle behaviours and preferences. These data will be used to compute your baseline Wellness Score. Based on your selected preferences, you will receive tailored health messages and reminders to engage in healthy lifestyle habits.
28. When do I need to complete the Onboarding Survey?
Please complete and submit the Onboarding Survey as soon as possible upon downloading the HealthTrack SG app to proceed with onboarding the programme.
29. Can I edit my responses to the Onboarding Survey?
You can edit your responses for up to 30 days after you have completed the survey.
30. Where can I view my responses to the Onboarding Survey?
On the HealthTrack SG app, tap on the menu (≡) at the top left, then select My Profile to view your responses to the Onboarding survey.
G. Blood tests
31. Do I need to undergo any blood tests*?
You will need to have blood tests* for glucose and cholesterol levels at 2 timepoints.
First timepoint
Blood tests* to be completed within 3 months before the first doctor's consult* for the programme.
Please refer to the table below for the corresponding blood tests required based on your chronic condition:

Important: If you have blood test* results completed within 3 months prior to first doctor’s consult, you do not need to repeat them. However, all blood tests* recommended for your condition must be completed. Outstanding blood tests* will need to be done before your consultation*.
For example, if you have prediabetes but have only completed the HbA1C and Fasting Blood Glucose tests* within 3 months prior to your doctor’s consultation*, you will still need to complete the Lipid Panel blood test*.
Second timepoint
Blood tests* to be completed within 1 month before the final doctor's consult* at the end of the programme. These blood tests will be arranged by your doctor, prior to your final consult for the programme.
During the 1-year programme, your doctor may arrange for additional blood tests* based on his/her assessment of your condition.

H. Doctor consults & bookings
33. How do I book a consult to see the doctor?
You may book your doctor consult* through the following methods:

34. How often do I need to see the doctor as part of the HealthTrack SG programme?
You will see the doctor at least twice - once at the start of the programme and once at the end of the 1-year programme.
During the 1-year programme, additional appointments* may be arranged by your doctor based on his/her assessment of your condition.

35. Can I transfer my healthcare provider to another clinic?
If you are currently seeing a doctor at Fullerton Health and wish to change to another Fullerton Health clinic, please contact Fullerton Health at : +65 6333 3636 between 8.30am-5.00pm.
Note: Hours may vary slightly by clinics. Please check with your specific clinic for operating hours.
Fullerton Health is the appointed healthcare provider for HealthTrack SG. All participants are required to complete their initial onboarding and final doctor consultations with a Fullerton Health doctor. Should you choose to discontinue follow-up with Fullerton Health, your participation in HealthTrack SG will also come to an end.
I. Report Health Status
36. What is Report Health Status?
The Report Health Status collects self-reported data on lifestyle factors (e.g. diet, smoking, alcohol and stress levels) and clinical factors (e.g. weight, waist circumference, blood tests [if any]). These results will be used to update your Wellness Score.
The Report Health Status can be found under 'Workspace'.

37. When do I need to complete the Report Health Status? How do I fill it in?
You will be notified to complete the Report Health Status every two weeks. The notification will appear at the top of your HealthTrack SG app upon opening the app. Please fill it in when you receive the notification.
On the HealthTrack SG app, tap on 'Workspace', 'Health Logs', and select 'Report Health Status'. Tap on 'Report Status' to complete the questionnaire, then click 'Save' at the end. You may refer to your responses by going back to 'Workspace'.
38. What happens if I forget to complete the Report Health Status (RHS)?
You have up to 7 days to complete each RHS. If you miss the 7-day window, you will not be able to go back and do it – you will only be able to complete the next RHS.
You will be rewarded for completing the RHS consistently (refer to Section K).

J. Wellness Plan
39. What is the Wellness Plan?
The Wellness Plan is a personalised care plan co-developed by you and your doctor. Based on your profile, preferences and health data, your doctor will recommend up to three lifestyle actions for you to try.
40. Where can I view the Wellness Plan?
After your doctor issues your Wellness Plan, you will receive a notification on the HealthTrack SG app. You may also go to the “Records” tab to view and download a copy of your Wellness Plan.
K. Rewards & incentives
41. Are there any incentives for participation in HealthTrack SG?
You may receive up to total of $140 worth of rewards for participation in HealthTrack SG.

42. How will rewards be given?
Rewards will be given in the form of HPB eVouchers via the HPB Healthy 365 app. HPB eVouchers have an expiry of 3 months from the date of issuance.
43. How will I know when the rewards are disbursed?
An email will be sent to the provided email address upon disbursement of the rewards into your Healthy 365 account.
L. Data confidentiality
44. How will HPB protect my data and ensure my privacy?
HPB places paramount importance on the protection of your data and information. All data and information provided by you and collected during the programme will be managed in accordance with the guidelines stated in HPB's Privacy Statement (refer to https://www.hpb.gov.sg/privacy-statement for more details).
45. Who will have access to my data?
The use of, disclosure of and provision of access to data and information provided by and collected from you will be in accordance with HPB's Privacy Statement.
HPB will require your consent via the Programme Participant Agreement (PPA) (https://www.hpb.gov.sg/healthy-living/healthtracksg/ppa) to allow HPB access to the relevant data and information before you participate in HealthTrack SG. If you are participating through your employer, please note that HPB may share an aggregated corporate health report with them. No individual or personally identifiable information will be shared.
Please note that data collected via the Fitbit and Connected Life will be shared with your Fullerton Health healthcare provider and HPB, with your consent. Relevant information will also be shared by your healthcare provider with HPB.
